Wellness Program

Frequently Asked Questions about our Wellness Program:

  • Do I need to get a wellness exam completed?
    If you are an employee on one of our group health insurance plans, you are required to get a wellness exam completed each year.
  • When is my wellness exam form due to HR?
    For 2025, exam forms must be turned in to HR by December 12th. If you do not do so, you will pay a 10% higher premium in 2026.
  • Where do I go for a wellness exam?
    You can choose to go to any doctor for your wellness exam. The physician is required to sign the wellness form, but please do not include any results on the form. That is HIPPA information that we do not need.
  • What if I get charged for a wellness exam?
    If you receive an invoice from a provider for blood tests or services completed as part of your wellness exam, the first step is to check with the provider to make sure that is was coded correctly. If they say that it was, then contact customer service on the back of your health insurance card. If you are not able to get a resolution at that point, contact HR for additional assistance.
  • Where can I get a wellness exam form?

  • What are the benefits to getting a wellness exam done?
    Wellness exams are free of charge to the patient. This is a great time to get bloodwork done, which for some is required in order to refill a prescription. Early prevention and maintenance of a chronic condition are huge benefits, too. This is also a great time to update your health records with your doctor and ask any questions that you may have that you generally don’t do during a visit when you are sick or injured.
  • Do I have to wait 365 days between wellness exams?
    We have calendar contracts with our programs, which means one exam per year. However, that does NOT require you to wait 365 days between wellness exams. If you doctor tells you that you need to wait, you can refer them to this website.
